The Certification of Trust Template is a formal, fillable PDF used to provide third parties with proof of a trustee’s authority without disclosing the full trust agreement. Financial institutions, title companies, investment firms, and other third parties commonly require a certification of trust before allowing a trustee to open accounts, transfer assets, or complete transactions on behalf of the trust.

This template includes standardized sections to identify the trust, grantor(s), and current trustee(s); describe trustee powers and authority; confirm successor trustee provisions; state revocability status; address co-trustee authority; and identify governing law. It also contains third-party reliance language and a notary block to support acceptance by banks, title companies, and other institutions. The form is designed to be customized to meet varying state statutory requirements.

Ideal for use during trust funding, incapacity administration, post-death administration, and ongoing trust management, this template helps trustees efficiently demonstrate authority while preserving the privacy of the trust’s full terms. The fillable PDF format allows trustees to complete the certification electronically, print for execution and notarization, and retain it as part of the trust’s permanent records.
